Provide professional, comprehensive nursing care for patients in an acute care environment. In this position you will:

Qualifications:

Required qualifications for this position include:

  • Nursing degree/diploma upon hire
  • Bachelor’s degree in Nursing or higher within 6 years of hire.
    • A RN with 20 years of RN experience will not be required to obtain a BSN
    • Exceptions will be considered on a case-by-case basis at each facility by the most senior administrative nursing leader in consultation with the Human Resources leader.
  • 12 Months acute care experience working with the patient population similar to department of hire or completion of the specialty residency located at that facility.
  • Unencumbered Swedish Registered Nurse (RN) license
    • If the Registered Nurse holds a multi-state license through the Nurse Licensure Compact, the RN must adhere to the requirements of licensure in the new state(s) of hire
  • Current Basic Life Support (BLS) for Healthcare Provider card must be from the following authorized governing bodies: American Heart Association (AHA), American Red Cross (ARC), or Military Training Network (MTN) upon hire.

Job description

OVERVIEW OF POSITION:

Responsible for the oversight, management and optimization of all quality improvement, utilization management and care management activities as it relates to pre-admission, ambulatory case management, inpatient, social service, home health, health education, behavioral health and other health care delivery programs within Health Care Partners Medical Group.  Manages staff directly responsible for the day-to-day operations of the care management programs.  Contributes to the clinical, quality, financial and patient satisfaction outcomes of the region.  Works closely with all members of the Care Management team as well as members of Health Enhancement, Quality Improvement, Contracting and the Office of the Medical Director to ensure compliance with all regulatory requirements.

ESSENTIAL FUNCTIONS:

  • Identifies need for and participates in the development and implementation of care management and utilization management policies and procedures, and ensures compliance throughout the region.
  • Monitors the utilization of inpatient and outpatient services and ensures coordination of all ambulatory care management services with pre-admission, home health, health education and other ambulatory services.
  • Closely monitors and analyzes all inpatient and outpatient reports and identifies trends. Makes recommendations to the Vice President/Director, Regional Care Management regarding interventions to improve all resource management.
  • Facilitates team process problem resolution using HCP administration, Medical Directors, Contracting, Risk Management, Behavioral Health and health plans regarding complex patient issues. Implements the results of the collaboration process.
  • Oversees and monitors patient eligibility, financial contract accountability and health plan benefit determination for each patient referral. Facilitates resolution on areas of conflict.
  • Prepares the department for accreditation surveys using the appropriate standards of performance. Ensures utilization management compliance with DMHC/CMS/NCQA requirements.
  • Leads activities related to the utilization review process in conjunction with the local Care Management Committee Provider Chair.
  • Designs, implements and facilitates data and statistical collection including outcome and quality of care information.
  • Monitors all high risk/high cost patients in regard to care delivery, referrals, contracting, etc. Monitors provider referral patterns for appropriate utilization of specialty and ancillary services.
  • Provides supervision and guidance to Supervisor and staff reporting to the position.
  • Conducts monthly staff meetings including utilization management review outcome measurements and identification of training/educational opportunities.
  • Identifies, develops and oversees the educational needs of Care Management staff, providers and others. These include an extensive orientation program, cross-training and proactive approach to case management.
  • Identifies opportunities for the development of new care management approaches and prepares proposals with cost analyses, IS requirements, education and implementation plans.
  • Contributes to the Care Management business plan and budget process along with the Vice President/Director, Regional Care Management.
  • Coordinates or performs projects/activities as delegated by organizational committees and Vice President/Director, Regional Care Management.
  • Performs additional duties as assigned.

EDUCATION:

  • Bachelor’s degree from a four-year college and/or a professional certification requiring formal education beyond a two-year college.
  • Graduate from an accredited school of Nursing.
  • Current California RN license.
  • Bachelor’s degree in Nursing or related field preferred.

EXPERIENCE:

 Minimum:

  • Over 3 years and up to and including 5 years of experience.
  • 3 to 5 years prior acute nursing experience.

Preferred:

  • 3 to 5 years prior acute nursing with critical care experience.
  • 2 to 4 years managed care/HMO experience in utilization review, case management or discharge planning.
  • 2 to 3 years management experience in utilization review, case management or discharge planning.

Function

The Hemodialysis Registered Nurse provides acute hemodialysis care and treatment, including developing initial and ongoing assessments, and providing patient training and education as applicable.

He/she must comply with the standards outlined in the policy and procedure manual, as well as the regulations set forth by the company and any applicable state and federal agencies.

Representative Duties

The Hemodialysis Registered Nurse will be responsible for the performance of all the duties listed. The distribution of effort between the duties listed can be expected to vary and will be under the supervision of a Hospital Administrator (HA), Group Hospital Administrator (GHA) and the Senior Director of Acute Operations.

  • Demonstrates high level of knowledge and experience skills and competencies related to the Hemodialysis Registered Nurse.
  • Demonstrates ability to perform technical skills for the Hemodialysis Registered Nurse including the patient’s treatment, water treatment and preparation, and emergency procedures as determined by the competency assessment program.
  • Delivers care to assigned patients as prescribed, including all aspects of the hemodialysis procedure in accordance with state requirements.
  • Demonstrates ability to work with any patient or staff and does not discriminate on any basis, including, but not limited to, race, gender, disease process, lifestyle and religious or cultural beliefs.
  • Initiates, monitors and discontinues dialysis treatments according to established procedures. Assesses patients pre/post treatment and reports problems to the Clinical Coordinator (CC) and/or Charge Nurse.
  • Establishes priorities of care, both emergent and non-emergent.
  • Develops and assimilates theoretical and technical knowledge with observations, assessments, and communication skills to detect dialysis and non-dialysis related problems.
  • Recognizes and utilizes opportunities for patient and family education and training.
  • Maintains a safe environment for self, patients and staff by utilizing proper body mechanics, and following universal infection control precautions and cleaning procedures under OSHA regulations.
  • Dialyzes isolation patients utilizing isolation precautions and prescribed procedures.
  • Assesses patients utilizing isolation precautions and prescribed procedures.
  • Performs hemodialysis access care according to established procedures.
  • Administers and documents medications as authorized and documented in patient’s health record by the physician and further authorized or limited by the State Board of Nursing Practice.
  • Reports any significant information or change in patient’s condition to the Clinical Coordinator (CC) or Charge Nurse.
  • Documents clearly, completely, and accurately all pertinent information during downtime of the electronic record.
  • Participates in the formulation, update, and revision of nursing care plans, including long and short-term goals, according to established guidelines.
  • Performs or assists in the performance of all emergency operational procedures, including the evacuation of patients in the event of a fire and/or ordered mock drills.
  • Obtains blood samples and other specimens as ordered and as outlined in hospital’s policy and procedure manual.
  • Initiates and discontinues blood transfusions and monitors patient progress per established policy in the hospital (if applicable).
  • Cooperates with and supports other departments in the hospital and follows all policies and procedures in the hospital (if applicable).
  • Assists with and performs all procedures of safe operation, disinfections and inventory control according to corporate and departmental policy, manufacturer recommendations and regulatory agency guidelines.

  • Picks up and delivers supplies, blood samples, records, and reports as directed by the Clinical Coordinator (CC) or charge nurse.
  • Assumes administrative duties under the direction of the Clinical Coordinator, Charge Nurse, or Hospital Administrator (HA).
    • Assists in orientation and training of new and existing staff.
    • Assists in CQI projects with the health care team.
    • Communicates any water, equipment, or facility problems quickly and appropriately following facility guidelines for reporting.

    • Acquires information on the latest developments of dialysis technique by participating in scheduled in-service classes.
    • Seeks resources for direction, when necessary.

Job description

Manages the delivery of nursing services and safe patient care within assigned department and across the continuum of care. Manages the delivery of services that add value and are integrated with nursing standards of practice, service priorities and performance/outcome measures, as well as with strategic, business and organizational goals/objectives.

Essential Responsibilities:

Manages on a 24 hour basis the delivery of nursing services and safe patient care which meet or exceed cost, quality, and clinical and utilization standards and performance measures.

  • Establishes, implements, and maintains patient care and service standards to meet members’ and internal clients’ expectations and needs in a changing, competitive health care market. Holds staff accountable in providing the highest quality of care while in compliance with the Nursing Practice Act, TJC, federal, state, and local requirements.
  • Works with Nursing Leadership to develop strategic/business plans to achieve integrated services across the continuum of care. Manages and ensures continuous improvement of all clinical practices, services and operations by designing and implementing systems, processes and methods to evaluate and improve patient care within assigned department and across the continuum of care.
  • Develops and monitors department’s budgets for the appropriate use of human and material resources.
